How to become a patient and what to expect as a new functional medicine patient -

If you think it's right for you - create an account & you will be sent intake forms, and FM specific intake questions and over 300 symptom evaluation-
You will need to submit a record release or provide the last 3 years of medical records.
Once this is received - it is thoroughly evaluated in conjunction with your intake information from an allopathic AND a functional medicine perspective.
(If you have not been seen by your PCP in the past 3 years - we start with a basic traditional medicine workup)
THEN you schedule your Functional Medicine Intake Appt 60-90 minutes.
Additional questions, explanations & a physical exam is completed.
The evaluation is shared with you at this point, any necessary labs are ordered & suggestion for specialty labs that will be the most beneficial is discussed ; preliminary lifestyle changes are suggested & any appropriate interventions to implement at this time provided.
Treatment plan projections are provided -
Once necessary diagnostic results have resulted & been reviewed you come in for Review of Findings appt. 60 minutes. This is an in-depth explanation of all results, how they got that way & how to fix it. - what the benefits of repair are and how many steps this needs to be addressed in & what the recommendations are
Evaluation of Progress appts will follow at intervals determined by the treatment plan that is built for you - or at any time you feel you need reevaluation this repeats until you feel like yourself again-
Then when it's appropriate we enter the maintenance phase.
There are no rigid algorithms - No two treatment plans are the same - and if there are similar treatments in two patients the responses will be different.
